I hereby grant non-exclusive permission for Goodwill Industries of Sacramento Valley & Northern Nevada, Inc. to use my photographic likeness in all forms and media for advertising, trade, and any other lawful purposes, including but not limited to: 

  • display on the internet and website
  • advertisements, documentation, and print collateral
  • training materials
  • broadcast (television), promotional videos
  • public relations and press materials
